Hi, I'm Jake. I'm a New York State community & 911 Paramedic with experience helping individuals and families navigate healthcare, wellness concerns, and daily support needs. Through my recent years especially as a community paramedic, I have noted a large gap with assistance for elderly in the home or their families. Besides being a paramedic I wanted to help more. Through Frontline Care & Response, I provide non-emergency welfare checks, patient advocacy, appointment assistance, healthcare navigation, family updates, and compassionate support for seniors, individuals with disabilities, and those recovering from illness. My goal is to provide peace of mind, promote independence, and help clients feel heard, supported, and connected to the resources they need.
