Following duties/Assignments I performed sincerely for past 2+ years for almost 15 patients/residents in my 8 hr shifts: *Observed and documented patient status and reported patient complaints to the case manager ; Linnea Ramos *Assisted with adequate nutrition and fluid intake. *Read and recorded temperature, pulse and respiration. *Planned, prepared and served meals and snacks according to prescribed diets. *Provided assistance and companionship to clients. *Performed household tasks such as laundry, dusting, washing dishes *Facilitated games and other activities to engage clients. *Assisted with transferring residents in and out of wheelchairs and adaptive equipment. *Positioned residents for comfort and to prevent skin pressure problems. *Tended to patients with chronic illnesses. *Charted daily information on the residents such as mood changes, mobility activity, eating percentages, and daily inputs and outputs. *Assisted with ADLs. *Provided patients and families with emotional support. *Exhibited compassionate care and communication with regard to issues of death and dying. *Administered simple range of motion exercises. *Promoted personal and co-worker safety. *Maintained a clean, orderly environment.
