Hi im a Licensed CNA providing 24hr care for Hospice patients & seniors returning Home from any Rehab , illness, surgery, or hospitalization. My focus is helping clients remain in their home while regaining strength, mobility, and independence & safely returning to their daily routines. I assist with prescribed Physical Therapy (PT) and Occupational Therapy (OT) home exercise programs every day 3x's per day while providing 24hr care, My goal is to assist your loved ones with regaining independence & assisting with bed mobility, transfers, walker-assisted ambulation, and activities of daily living. Personal care assistance such as bathing, hygiene, grooming, hair care, medication reminders, meal preparation, light housekeeping, and structured daily routines. I provide short-term recovery assistance, long-term care, 24-hour support, I also provide my own Bed alarm for fall risk safety clients & I provide a Call button to press if & when assistance is needed in-Home

We found Chasity when my 86 yo mom was in the hospital and needed extra care when she got home. Chasity made herself available to meet with us right away, offered lots of suggestions to make living with a caregiver easier and even brought her own ringer that mom could use when she needed her. She was very willing to help around the house and adjust hours when our needs changed. She was always reliable and on time. We’re so grateful to have found such a conscientious caregiver! :)
